Applicants must submlt a copy of picture identification or have their request notarized and provide a family tree sketch to illustrate the family connection and possibly provide proof of family connection such as a birth certificate. Tight restrictionson who can have copies. A family member must complete and sign an application available from the division. When requesting please Include the full name, father's full name, mother's full maiden name, date of birth and city or county of birth. The certificate must be your own or that of an immediate family member. Include reason for request and relationship to person on certificate.
